Incident 1768: Kenya's AI Health Insurance Means Test Reportedly Overcharged Poor Households and Obstructed Care

Description: Kenya's Social Health Authority reportedly used a machine-learning means test to set health insurance premiums. Investigators alleged that income overestimation burdened poor households with unaffordable charges. A Nairobi laundry worker was reportedly sent home before surgery and could not afford follow-up care. The technical audit used a reconstruction; the exact deployed model and implementation of proposed changes were not confirmed. SHA defended the model as equitable.
Editor Notes: 10/01/2024: National SHA rollout began; used as the deployment-period anchor, not a verified first harmful assessment. 08/2025: Africa Uncensored requested model details. 11/2025: SHA disclosed the formula and variables. 05/04/2026: Joint investigation and technical methodology published. 05/06/2026: The Elephant republished the investigation; SHA defended the tool. 05/07/2026: Investigators said key questions remained unanswered. 10/19/2026: Incident ID created.

Variants

A "variant" is an AI incident similar to a known case—it has the same causes, harms, and AI system. Instead of listing it separately, we group it under the first reported incident. Unlike other incidents, variants do not need to have been reported outside the AIID. Learn more from the research paper.
