JEFFERSON, Ga. --- Marci Bakely couldn't understand how her ex-boyfriend always seemed to know where she was.

When she went to buy groceries or on a date or to the doctor, he'd text her about it, sometimes within minutes, despite working 15 miles from her home as a police chief in a small town in the Atlanta suburbs. One day in 2024, when he incorrectly suspected that she'd gone to see a man against his wishes, he messaged her, "I didn't think you would slip up," according to texts she shared with The Washington Post.

Bakely, 50, began to feel like she was constantly being watched. She says

Protest over alleged sexual deepfakes of female students ends with vandalism and police presence at a school in Puente Alto

Incident 1757: Students Allegedly Used AI to Create and Share Fake Nude Images of Classmates in Puente Alto, Chile

“Protest over alleged sexual deepfakes of female students ends with vandalism and police presence at a school in Puente Alto”
adnradio.cl2026-10-15

A protest by high school students at Colegio Alicante del Sol in Puente Alto ended this Thursday with vandalism, clashes, and the presence of the Carabineros (Chilean police), amidst the anger of students and parents over a case of alleged deepfakes involving sexual content that has been ongoing since July.

According to information gathered by ADN.cl, on July 10, students allegedly discovered sexually explicit images on a classmate's phone, created using artificial intelligence, that reportedly involved female students from the school.

After the situation came to light, which is being investigated by the Public Prosecutor's Office, the families of the alleged victims went to the school, and the students involved were suspended pending an investigation.

However, according to testimonies displayed on posters inside the school, the students later returned to classes, although assigned to different courses. This decision angered some students, who questioned the school's response and demanded more action in the matter.

Aggression and Arrival of Police

Tensions escalated following further incidents reported by students and their families, who claim to have been mocked and threatened in the context of the conflict.

The conflict escalated around 11:30 a.m. on Thursday, August 27, when students held a demonstration that was initially reported as peaceful. Later, damage to windows, the activation of a fire extinguisher, and clashes were reported.

According to information obtained by ADN, police arrived at the school shortly after noon, following the preliminary identification by their classmates of three students as the alleged perpetrators of the leaked images. Fearing for their safety, the students took refuge in a classroom. Police and municipal personnel subsequently entered the school and safely removed them from the premises.

The school confirmed that the protest resulted in disturbances, physical abuse of staff and students, and damage to the building. Furthermore, at the request of the Provincial Education Authority, the school dismissed students early and suspended classes for Friday.

The Parents' Association, meanwhile, called an urgent meeting to address the situation and coordinate the next steps.

What did the authorities say? -----------------------------

Once the events were known, the mayor of Puente Alto, Matías Toledo, stated that he went to the school along with the Carabineros (Chilean police), Municipal Security, and the Provincial Delegation, following the student protest.

He explained that the students involved in the incident were taken to a police station as part of the procedure, after attempted assaults occurred during the protest.

Toledo maintained that, although the school had taken measures and filed the corresponding reports, the situation escalated after the students involved returned home.

The mayor also called on parents, guardians, and students to consider the consequences of the irresponsible use of artificial intelligence and affirmed that the municipality is committed to working with the educational community and families to find a solution that will prevent future incidents.

Lawsuit claims Sharp HealthCare secretly recorded exam room conversations without patient consent

Incident 1754: California Health System Allegedly Used an AI Medical Scribe to Record Patients Without Consent

“Lawsuit claims Sharp HealthCare secretly recorded exam room conversations without patient consent”
kpbs.org2026-10-15

A proposed class-action lawsuit filed in San Diego Superior Court claims Sharp HealthCare secretly recorded doctor-patient conversations using an artificial intelligence tool --- and falsely documented that patients had given consent.

Sharp began using an AI app called Abridge in April of this year, according to the complaint. The tool records conversations between patients and clinicians and then generates draft medical notes. According to the lawsuit, the app captured everything said inside the exam room --- including symptoms, diagnoses and personal details --- and sent the audio to the co

About the Database

The AI Incident Database is dedicated to indexing the collective history of harms or near harms realized in the real world by the deployment of artificial intelligence systems. Like similar databases in aviation and computer security, the AI Incident Database aims to learn from experience so we can prevent or mitigate bad outcomes.

You are invited to submit incident reports, whereupon submissions will be indexed and made discoverable to the world. Artificial intelligence will only be a benefit to people and society if we collectively record and learn from its failings. (Learn More)
